2013 Mail Order Information

Mail-Order Prescriptions Available
As a member of Senior Care Plus, you have the option of receiving some of your covered prescription medications through mail order. Senior Care Plus members can receive mail-order prescriptions from Catamaran Rx. If you take maintenance medications to treat chronic conditions such as arthritis, diabetes or high blood pressure, your benefit may provide a 90-day supply through mail order. Please refer to your Evidence of Coverage for more information.

Ordering new mail-order prescriptions 
Catamaran Rx provides mail order prescription service, for your Senior Care Plus pharmacy benefit.

Please ask your physician to write a new prescription. Then submit it to Catamaran using the instructions below. You must enroll with Catamaran before Catamaran will refill any mail service prescriptions. To avoid disrupting delivery of your transferred prescription refills, please enroll by one of these methods.

To enroll by mail, complete a Registration and Prescription Order Form; include the member ID and Rx group numbers that are printed on your member ID card, and return the form in the preaddressed, postage-paid envelope that Catamaran will provide.

To enroll online: o
n the home page, click on Enter Member Site; on the Member page, click on Prescription Medications on the left side of the screen; on the Prescription Medications page, scroll to the bottom and click on Catamaran Rx Mail-Order Service and follow the instructions for new enrollment. Upon completion, your enrollment will be processed within two business days. To enroll by telephone, call Catamaran Rx at 888-341-8574, seven days a week, 8 a.m. to 8 p.m., Pacific Time.

To request a refill of a transferred prescription, call Catamaran Rx at 888-341-8574. Transferred prescriptions will not be mailed automatically. If you choose to enroll by telephone, a customer service representative can initiate your refill at that time. Allow at least 14 days for shipping and delivery of your prescriptions from the time that Catamaran receives your request.

After you enroll, you can mail the Registration and Prescription Order Form to Catamaran along with a new prescription for any medication that you wish to receive through mail service. Ask your physician to write the prescription for up to a 90-day supply and include the number of refills for up to one year, if appropriate. If you cannot obtain the prescription from your physician promptly, ask your physician to fax the prescription directly to Catamaran using an Catamaran Prescription Fax Form. The physician's office must fax the Prescription Fax Form directly to Catamaran. You must have a 30-day fill of that prescription at a retail pharmacy before you may use the mail service.

Once you are enrolled and have established your prescriptions with Catamaran, you may obtain refills: by completing a refill request slip (included with every order) and mailing it to Catamaran; by calling 888-341-8574 to access an automated refill line or to speak with a customer service representative to order your refills directly; by registering for Catamaran online Member Services.

isit the Catamaran Web via hometownhealth.com using the same navigation as when you registered with Catamaran; please allow 48 hours for Catamaran to activate your online account before attempting to use the online refill function; please remember to order your medications at least 14 days before your supply runs out, to allow time for shipping and delivery; and include the appropriate copayment amount with your order. You may pay with: Visa, MasterCard or Discover card; Personal check; or Money order. Please do not send cash.
