Who do I call/email if I have a question on my bill?

Please find the person assigned to the first letter of your company name in the Premium Accounting section of the Personnel Contact Sheet.

Who do I call/email if I have a question about eligibility or enrollment?

Please find the person assigned to the first letter of your company name in the Enrollment section of the Personnel  Contact Sheet.

Who do I call/email if I have a question about IChoose?

Please call your external Sales Service Representative listed on the Personnel Contact Sheet.

Who do I call/email if an employee has a question about claims, authorizations, network, etc?

Please direct them to log on to the member portion of this website or have them call Customer Service a 775-982-3232.

Who do I notify of desired changes in provisions during open enrollment?

Contact your Account Manager or external Sales Service Representative listed on the Personnel Contact Sheet. Please refer to the Eligibility Guidelines to determine what documentation is needed.

Who do I contact with questions on COBRA?

If you are signed up for Infininsource, please contact them at 800-300-3838.

Who do I contact with questions on State Continuation of Benefits?

Please contact the State at 775-687-4270 for information.

When do I contact my Account Manager?

If you have questions driven by member issues.

If you need Open Enrollment meetings or pre-enrollment packets.

If you're not sure who else to call!

What if I can't reach my Account Manager or Sales Service Representative?

Both postions require much time away from their desks, so please use the main Marketing line at 775-982-3100 or contact your internal Sales Service Representative on the  Personnel Contact Sheet.

Where do I access PDF forms such as enrollment forms, waivers, medical assessment forms, etc?

Log into iChoose and go to "forms" section.  If you have not been set up for access to iChoose (our electronic enrollment web-site) please contact your external Sales Service Representative.  Please refer to the Eligibility Guidelines for information on what forms are needed.
