New in 2019, Hometown Health plans offer a Gym Benefit for our members*. Hometown Health members with qualifying plans who would like to use their gym benefit MUST first complete the Health Risk Assessment (HRA) on the Healthy Tracks website.
If you are a current Healthy Tracks user, please log into your Healthy Tracks account. Complete the Health Risk Assessment and select “yes” when reviewing the gym benefit Terms and Conditions.
Don’t currently have a Healthy Tracks account? It only takes a minute to register your account. You will want to have your Hometown Health ID card and member number on hand. Click here to register your new Healthy Tracks account. Once you complete the HRA and accept the terms and conditions, you can visit one of the Participating Gyms to access your Gym Benefit.

Note: After you complete the HRA, it will take 24 hours before the Gym can confirm eligibility.
*The Gym Benefit does not apply to all Hometown Health Plans. Individual and Family Plans and Small Employer Group Plans effective in 2019 have this added benefit.
