2024 Member ID Card Update

Members will receive ID cards prior to 1/1/2024 with the new OptumRX information listed and their current plan information. If the member has not renewed yet or is in the process of renewing, another set of ID cards with the new plan information and OptumRX information will be mailed.

The most current ID card will be available to be accessed in the members MyChart account.

NOTE: Please present your Hometown Health membership card and pay any copayment due at the time you receive services. If you fail to present your membership card or notify the provider that you have Hometown Health coverage, you may be responsible for paying for the services rendered during that visit.
