Changing Your Primary Care Physician

If you need to change your Primary Care Physician, you may do so several ways:

1. By phone at 775-982-3232
2. By fax at 775-982-3741, Attention: Customer Service Department
3. Or by e-mail: customerservice@hometownhealth.com

You will receive a new membership card in the mail approximately seven (7) days from the date that you make your Primary Care Physician change with Hometown Health. Please be sure to destroy your old card and present the correct membership card to your new Primary Care Physician. We suggest that you contact your former Primary Care Physician’s office and ask them to transfer your medical records to your new Primary Care Physician.
