Qualifications and Criteria for the Carson City Chamber of Commerce Association Health Plan:
  • Must have a valid Nevada State Business License.
  • HMO and PPO plans are available to businesses
    located in Carson City, Douglas, Lyon, Storey and
    Washoe counties. EPO plans are available to
    businesses located statewide except for those
    located in White Pine and Elko counties.
  • Available to manufacturing and service
    businesses with 2-50 employees. Qualification
    is based on your client’s NAICS code. Reach
    out to your account specialist to see if your
    client qualifies.
  • Each group is subject to underwriting review.
    Provide a complete census with employee DOB,
    home zip, and gender for underwriting review.
    Medical questionnaires are not required!
  • Group must be a member of the Carson City
    Chamber to officially enroll.
