Hometown Health is pleased to partner with Friday Health Plans effective January 1, 2021. Friday Health Plans will be leasing Hometown Health’s PPO network. They will utilize Hometown Health’s PPO contracted rates and Hometown Health will be pricing claims based on those rates. If you are contracted with Hometown Health, you are able to accept and provide care to Friday Health Plan members as an in-network provider. We encourage you to register at Friday Health Plans Provider Portal to view member eligibility and claim information.

Possession of the ID card does not guarantee eligibility of coverage. Providers must verify prior to services being rendered for non-urgent or non-emergent.
If a Specialist will be performing any procedures, they must request authorization by submitting a referral directly to Friday Health Plans

No. It is not necessary to contract directly with Friday Health Plans. This client will utilize Hometown Health’s PPO contracted rates.

Friday Health Plans will be handling all member services. Hometown Health will only be pricing claims.
