Your Membership Card

We have made the following updates to your member ID card:

  • Your network is now printed in the top left-hand corner of your member ID card for easy reference when searching our Find a Doctor online directory.
  • We have removed co-pay amounts from the ID card. To view your co-pay information, please login to MyChart.
You will receive a letter explaining the changes to the member ID card when you receive your new Hometown Health member ID card.
Please note that for the 2021 Plan year, the Signature Plan is now the Renown Health Plan, brought to you by Hometown Health. You will receive a membership card for each family member enrolled in Hometown Health. Please contact our Customer Service Department if you misplace your card.

NOTE: Please present your Hometown Health membership card and pay any copayment due at the time you receive services. If you fail to present your membership card or notify the provider that you have Hometown Health coverage, you may be responsible for paying for the services rendered during that visit.
