Determine Authorization Requirements for a Specific Service
The easiest way to determine authorization requirements for a specific service or procedure is to review your Evidence of Coverage. Follow the steps below to locate the Evidence of Coverage document for your plan. You can then review to determine authorization requirements for a specific services.
Step 1: Navigate to the Plan Documents page using the link below.
Step2: Once on the plan documents page, select the Evidence of Coverage for the plan you are currently enrolled in.
Step 3: You will find services that require Prior-Authorization are described in Chapter 5, Section C of the Evidence of Coverage document.
